Die Hand des chronischen Polyarthritikers: Pathomechanismus von Subluxationen und Präventionskonzepte

Summary

On the basis of a vast number of references and a synthesis of so far known morphological peculiarities and functions of the great variety of articulations of the hand, ideas concerning therapy and prevention are developed with regard to the origin of typical subluxations and deformations of the chronic polyarthritic's hand. In introductory remarks, the weak points in bone, ligaments and muscles of the normal hand as well as principal structural damage due to rheumatic inflammation and tumorous pannus are compiled. Interest is focussed on the physiological function of the proximal carpal region and possible variations of pathologic function. Possible and frequent subluxations, mainly of the lunate bone, but of the whole proximal wrist as well, may be the beginning of so-called zigzag deformations, which are dealt with in detail. The descriptions of preventive programs always represent the result structural and functional considerations of the hand.

Zusammenfassung

Aufgrund einer großen Literaturübersicht und einer Synthese der bis heute bekanntgewordenen morphologischen Besonderheiten und Funktionsweisen der zahlreichen Gelenke der Hand werden thérapie- und präventionsrelevante Vorstellungen zur Entstehung der typischen Subluxationen und Deformitäten der Hand des chronischen Polyarthritikers entwickelt. Einleitend werden die knöchernen, ligamentären und muskulären Schwachstellen der normalen Hand sowie die prinzipiellen strukturellen Schäden durch die rheumatische Entzündung sowie den tumorösen Pannus zusammengestellt. Im Mittelpunkt stehen die physiologische und die möglichen Varianten einer pathologischen Funktionsweise der proximalen Handwurzelreihe. Die möglichen und häufig sich einstellenden Subluxationen hauptsächlich des Os lunatum, aber auch des gesamten proximalen Karpus, können der Beginn von sogenannten Zickzackdeformitäten sein, auf die besonders eingegangen wird. Die Beschreibungen der Präventionskonzepte stellen stets das Resultat der strukturell-funktionellen Betrachtungen der Hand dar.
